Support Teacher Transition (STT)- Department of Education

Support Teachers Transition are specialist teachers who assist students with disability and additional learning and support needs.  STTs support high school students in mainstream classes, support units and special schools across all Department of Education settings.  They help students, their teachers, parents and carers with planning the transition from school to adult life.

STTs undertake a range of activities that may include:

      • providing advice on curriculum options and subject selection
      • providing advice on vocational preparation, work experience and entry to HSC Vocational Education and Training courses
      • assisting with transition planning to post school destinations which may include employment and further education, transport and mobility issues, recreation and leisure pursuits
      • liaising with a range of government and non-government providers
      • assisting students and their families to access specialist programs and support provided by other government agencies.

Your STT can assist you to understand post school options and assist in the compilation of documents (ITPs) that can support the NDIS review process.

Individual Transition Plan (ITP)

An Individual Transition Plan (ITP) is a holistic student-centred document created as a result of a collaborative transition planning process involving information sharing between students, parents and carers, schools and government and community agencies and services. Planning for leaving school, generally increases in the student’s final years of schooling, but usually begins earlier – depending on the student’s needs.
This collaborative approach helps students to make informed decisions about their future direction and provides links to local post school government and community support.
Planning for post school destinations may take into account the student’s:

• further education and training interests and needs
• employment pathways
• transport and mobility requirements
• leisure and recreation interests
• personal care and management needs
• community living options

Contact your school to organise a Transition Planning meeting with an STT.
